ABOUT THE OWNER
Joyce Sun, an experienced Registered Nurse and Owner of Hand in Hand Care Management, developed this company out of a personal desire to offer support and advocacy for you and your loved ones. She brings value to the care management process by utilizing over 20 years of nursing experience to guide you or your family member through a short-term crisis or a long-term chronic condition. She holds current nationally recognized certifications in case management and hospice/palliative care. Her experience ranges from acute hospital care to home/hospice care. She is also a certified dementia practitioner and understands the dynamics of dementia unique to the needs of each individual. Joyce is dedicated to providing you with an individualized experience aimed at promoting safety, autonomy, and independence.
Our approach centers on person-centered care goals. The goals of care are a result of collaboration with you, your family, and physicians. As your life or needs change, we want to support you in staying safe and independent.
